Pursuant to Section 19a-638 of the Connecticut General Status, CT Clinical Services, Inc. and the AMCP Treatment Intermediate, LLC will submit the following Certificate of Need (“CON”) Application:

Applicants: CT Clinical Services, Inc. (d/b/a Turnbridge) and AMCP Treatment Intermediate, LLC

Proposal: The applicants will file a CON Application with the State of Connecticut Office of Health Strategy for the approval of the transfer of ownership of CT Clinical Services, Inc. parents corporation, CT Clinical Partners, Inc., to AMCP Treatment Intermediate, LLC. CT Clinical Services, Inc. (d/b/a Turnbridge) will continue providing the same services at the same locations as a result of the proposal.
